Horizon Smile Orthodontics participates with most insurance plans for braces and orthodontics available on the market today. You will find us listed as “John Vondrak, D.D.S.” under the list of participating providers on your dental insurance company’s website.

If your insurance covers braces and orthodontic treatment, you will receive the orthodontic Lifetime Maximum benefit that your policy allows. Orthodontic insurance is very different from dental or medical insurance, in that it does not pay the claim in full upon the banding of the orthodontic appliance, but instead spreads the payments out over the course of orthodontic treatment (typically between 18 and 24 months, depending upon the provider). Based on enrollment periods, it will be necessary for you to maintain your premiums to keep your policy in effect for a minimum of two to three years. Should your policy terminate for any reason, the outstanding insurance balance will become your responsibility.

If your dental plan covers orthodontic treatment, you may receive a benefit for Invisalign. We will contact your insurance provider to determine the extent of coverage for your treatment.

As a courtesy to you, our patient, we will help you determine any orthodontic benefits you may have available, submit your claim, and handle any insurance questions that may arise on your behalf. But ultimately, we must emphasize that as your orthodontist, our relationship is with you, not your insurance company. In essence, the insurance is responsible to the patient and the patient is responsible to the doctor’s office.
